patients heads keep exploding
they are ungrateful of my treatment so they kill themselves before i finish

That is typically caused by lack of proper restraint, or the fragility multiplier is way too high. I would reccomend keeping all limbs and torso segments connected to the table with fixed cable, as it does not allow movment.
