is this clientside or does it replace the weapon all the same? can i use this client side without it effecting others?

pretty much every Terraria workshop item is clientside, including this one.

You have to modify the localization folder:

{
"ItemName": {
"CombatWrench": "Leviathan Axe",
},
}
