Have you seen the green player figure?
The first time I entered the map (day version) and died in a dark room, a green figure of Gordon Freeman appeared on the spawn (usually it indicates the place where the player spawns). I went up to it and it killed me, and then it just disappeared.

i had it too, i was posing some Strider corpse near spawn when it appeared in-between its legs and started teleporting around the spawn
