Pocket Dimension (as you know) is broken, teleporting to EDR Below the tower

I've never been able to replicate this happening. All I can really say is try adjusting the order in which mods are loaded in the mod menu. If it still continues I can try a more drastic solution.

Other people have reported this bug. I have no idea how to replicate it nor what is causing it, so I guess just refrain from taking the perk in the future, sorry. Best guess I can make is hardware or engine quirk preventing the pocket dimension scene from loading correctly.

The Pocket Dimension bug is because this mod relies on a library from the "More Stuff" mod. Install More Stuff, and Pocket Dimension will work.
