when doused - -20% max health for 10 seconds
alt-fire to eat (spy mains only) - grands all negative effects at once

So when you eat a spy doused in ♥♥♥♥ you now take minicrits twice, taking damage heals the enemy, you become 50% slower and you start to bleed?

nah, if a spy is covered he can eat the "substance" from himself and have all the negative effects, but also remove the health decrease.
