It seems when commons or other infected go trough the gate,this event starts.
I've fixed both the visual bug and the saferoom bar being missing. I left the windows non-solid due to it creating issues for bots. Especially special infected, having trouble traversing smoothly, but thanks for reporting it nonetheless.
First off, the easy fix: one of the cigarette machines is floating about 8 units off the ground. I don't think I noticed any other prop errors though. Nice work!
Now, for the labor-intensive suggestions. As a lazy mapper myself, I don't blame you for rejecting any of my crap ideas
- I have a novel suggestion for fixing the fire in the fire alarm crescendo event. Right now it magically pops into view as you're walking into the room, which is an unfortunate engine limitation. But one decent workaround you could try is adding a custom fire texture (an animated fire.gif preferably), putting it on one side of an otherwise invisible, non-solid func_lod, and setting the fadeout distance to make the func_lod disappear right after the particle pops into view. Better than nothing I think.
- It's unfortunate that there's little to no graffiti or dialogue, especially in the safe rooms. I know it doesn't affect gameplay (and talkative Survivors are a pain in the ass to set up), but I would at least recommend considering safe room graffiti. Valve added it to give players something to do in the safe room while they wait for their teammates to get ready, and I think that's an important consideration.
- In my opinion, Map 4 needs more work than the other three combined. I personally didn't have too much trouble with the hordes and Tanks, so I guess your first patch worked well there. However, I struggled to find out where to go several times because the map is incredibly dark and somewhat maze-like. There's a part near-ish the beginning where you're going around a chain link fence, and I got so confused I went backwards all the way to the start of the gauntlet. I would strongly recommend brightening up the map significantly and adding some arrows to point the way forward at forks and junctions. You could also try making the "passageways" tighter and more cramped -- less chance for Survivors getting lost, greater chance for zombies to surprise them, win-win. Getting lost for a bit is fine in normal gameplay, but it kills a gauntlet finale.

TL:DR - Just install Half-Life 2 (Well, I mean it worked for me.)
When installing both "Last Breath", and "Last Breath: Content", I noticed that the AppIcon is the default pink and black missing texture. I would jump into a game, and even though I still have both addons installed, it would still show missing textures. When I hop into my addons, it doesn't say the addons are conflicting with anything. I installed HL2 to see if it worked, and for some reason it did? I think it's like a Garry's Mod situation, where some addons require HL2, but I don't think that L4D2 mounts directly to HL2.
If this is happening to someone else, try to install that, or reply to me. I am curious to see if it is about HL2, or is it something bigger?

Sounds like an issue on your end. Try the map without any custom mods to see if it resolves the issue.
Did you have this issue on just the first level or all 4?
