How to get the Crazy Fish jumpscare
1. First of all, you can't just get it on your first try. It is decided by Crazy Fish itself. (Or RNG, that's probably the most likely option)

When you spawn in, check the TV. If you see Breen dead, then you know what you must do.
If you don't, then restart the map. This may take several hours.

2. Break all the windows in all 3 houses. This should be straight-forward.

3. *IMPORTANT* *HEALTH MUST BE AT RED LEVEL*
Lastly, take the boat, go to THIS edge, and wait for about a minute. The Crazy Fish will appear.

The game will crash after that.
