Is there any trick to enable this?
Hey, I've tried promoting a bunch of new admirals and generals as the UK and the USSR and haven't seen any of the new portraits. Is there something I'm missing?

You are missing the fact that this mod adds portraits for countries that do not have them in principle and they are taken from a certain list of generics. And the USSR and Great Britain have their own separate lists with portraits.

But portraits of leaders will work in all countries where there is no pre-created leader of ideology.
I will take this problem into account and add suitable portraits from the list for Majors.
