Report a translation problem
So at this point I'm at even more loss. The pillows are clearly unmodified, if not the pillow could it be some codes interering with the bed or smth? guess I'll get to the list asap :|
Now that is VERY strange, the code below is how comfy sleeping is checking for the nearby pillow:
All comfy sleeping does to check for a pillow is:
1. Check each tiles world items (the ones placed on the ground/dropped)
2. For each item, check it's name (the Base.Pillow part from above)
3. If the lowercase name that Comfy Sleeping finds contains the word 'pillow', then it returns true and will apply the comfort bonus.
The only thing I can think of thats causing the pillow check to break is something removing Comfy Sleepings event that waits for the context menu to fill with options?
There's not a whole lot I can offer to help, other than unsubbing and resubscribing to Comfy Sleeping. If you do have a copy of the mods you have (the config file for your server would also work, it lists all of your mods and their workshop ID's) then I could take a look on my own server and see what might be going on.
I will try resub comfy sleeping.
In the meantime I'm almost numbed after my test. Just like you said this mod isn't that complex so I'm quite dumbfounded for the result.
I was listing mods that might have some alternation regarding beds or pillows or sleeping, and before I handing out the list I wanted to test for a bit just for both of ours sakes. And dozens of mods was diabled, the pillow nearby is still no.
Of course I can't guarantee I've disabled all the right mods, but that's about all the mods I thought that would interfere with beds or pillows or sleeping.
So I'll have to agree what you said, there might be something messing with context menu itself, blocking comfy sleeping's event or whatever. I shall leave my modlist here minus the mods I've tested that didn't change the result.

I hate to say it in this occasion but, good hunting for both of us ;(
Either through the 'Host' button on the main menu or a dedicated server would work. If you could copy and paste the ''ModID:" and 'WorkshopID:" lines of your servers config file I can try spin my server up and itll be a lot quicker sifting through problematic mods.
I'd like to make clear that by "modid" and "workshopid" do you mean (server name).ini file under the C:\users\blablabla\zomboid\server folder?
C:\users\YourUserHere\Zomboid\Server\
And then in the YourServerName.ini file, there will be two lines somewhere in that file that look like this (may have to press ctrl + f to search for them):

If you can copy & paste the 'Mods' and 'WorkshopItems' lines here then I'll be able to open a blank Zomboid server with your mods in the same order, and I can try search & sift through them for the problematic mod.
The 'Mods' line should have all the names of the mods you pasted above, and the 'workshop items' line is all their corresponding workshop ID's.
Understood. I got the modid bit but didn't realized the latter being workshop items, and they're as follows:
Much appreciated, I'll get my server setup with a new config that has your mods and see if I can't sift through them and find the culprit.
If I find anything, I'll let you know here, if not, then it might be a mystery for a while unless someone else reports a similar issue :/
Being appreciative as I'm, I understand that the debug process isn't as easy as it sounds and it probably wouldn't be done in a minute so no rush nor worries.
In the meantime, I too shall try and disabled some of mods in the modlist, see if I can narrow the range down a bit more. The god of pz modding above, may good luck falls to both of us lolol
The good news is that nothing appears to be conflicting with Comfy Sleeping at all, including the mod that changes beds to have pillows ontop. Here's a screenshot with pillows on the bed that spawned naturally, and one after I picked up the nearby pillow:

Could you try loading up your current mod list on a new save? I loaded up your mod list on an old save of mine so it should behave correctly mid-save, but this is one of the last things I can think of to offer any help.
Other than that, maybe try multiple ways of dropping the pillow? As in right clicking it and clicking 'Drop', drag & drop it out of inventory, or placing it at your characters feet.
First of all I'd like to thank you for all your time and efforts for this matter. I know that debugging a bunch of potentially broken mods isn't gonna be not time consuming.
Then I'm happy to know the mod works for I really like this mod adding in immersion and reasonable difficulty. But the sad news is I tried what you said and the problem is unsurprisingly persistent.
However I did run some other mods that I hadn't listed eariler before, but I doubt these mods would break such a mod, because these mods are mostly translation mods for some of my co-players that speak difference languages, they're pretty much a bunch of text files so they shouldn't be able to do this?
And the other mods are just some customized tweaks of mods made by myself, even tho they do contain codes they're quite simple and primitive as I process next to no programming skills.
Now if you ask me I'll say the culprit is most likely hidden in these unlisted custom mods, considering that you've spent quite some time onto this mess, I shall tend to these mods myself before throwing any more challenge for you.
Well good thing is you did help narrow the list quite a lot so this wouldn't be too hard for me, hopefully lol I will try and sort these and report back worthy of note.
If those other mods are just translation files, then it shouldn't conflict with Comfy Sleeping at all. They just let the game know what text to use for sandbox option names/tooltips, and in game text prompts specific to that mod only though. So you're correct in assuming that translation files more than likely have no affect here.
No code from other mods should interact or block Comfy Sleeping at all. I thought about it earlier (regarding the whole 'a mod might be removing an event Comfy Sleeping needs') but if this were the case, then the 'Sleep' context option would remain unchanged from vanilla as it NEEDS the event to be there for Comfy Sleeping to update the text in the tooltip (to add your character comfort and the pillow nearby status when hovering over 'Sleep'). So this is also not the issue.
Don't mention it, it's a shame we can't still defeat this issue.
And I got more bad news already: I disabled all the custom mods and translation mods in an existing solo save and the pillows are as welcoming as always.
So again I'm in an indescribable loss, guess I'll have to disable them batch by batch to find the culprit then :(
It might be worthwhile to try just Comfy Sleeping on it's own in a singleplayer save. At this point, I'm not convinced your other mods are affecting it at all.
What language do you play in by the way? I wonder if somehow it is a language/translation issue. Maybe switch it to english if you don't play in english and see if the issue persists?
