I'm in the same situation. No other mods doing anything similar, settings seem good, but cannot use the mod at all in-game.

I've restarted, saved settings and everything. As soon as I load the game, its not working. Then i check the settings and its reverted back.

"Not Working" doesn't help at all. That's not necessarily an issue from the mod, it could be a symptom of server crashes in the past, network issues, permissions, problems with the Steam Workshop servers, or some other underlying problem. You can try to delete all files and folders inside Project Zomboid Dedicated Server\steamapps\workshop, this will force the server to redownload all mods.
Delete the following folders and files manually:

C:\Program Files (x86)\Steam\steamapps\common\Project Zomboid Dedicated Server\steamapps\workshop\appworkshop_108600.acf (file)

C:\Program Files (x86)\Steam\steamapps\common\Project Zomboid Dedicated Server\steamapps\workshop\content\108600 (folder)

And also upload the server-console.txt file. It can be found in:

Windows : C:\Users\%USERNAME%\Zomboid\server-console.txt

Mac/Linux : $User$\Zomboid\server-console.txt
