A Note On Canonicity
I'm not going to go region-by-region like I did for the IEE mod. So I'll just mention a few things.
Empire
-100% canon, I think
-Slashscratch Deep's location is a guess, it is most likely in the empire though
Bretonnia
-Oisillon, some oldhammer thing, stuck it in Gisoreux where there was room
-Languerre de Lac, city destroyed by Galrauch, location is a guess (can you tell Gisoreux doesn't have much going for it in canon.... :p)
-Raisol, which I got from Gundehar of Raisol, who is from Bordeleaux
-Garban Hold, yeah I know it should be on the other side, but there's room in the Pale Sisters
Tilea/Estalia/Border Princes
-I view Alfred Nuñez Jr.'s stuff as basically canon
-Valley of Windmills, location is a guess
-Skabreach, location is a guess
-The Great Mills, sort of made-up
-Tinea, location is a guess
-Wissenheim, from ‎Shadow of the Horned Rat
Badlands
-Based some of the settlements on local greenskin tribes
-Several "<Greenskin Tribe> Hold/Fort/Camp" I just stuck whereever.
-There's quite a few things from Warhammer Online, like Sharpthorn Forest
-Razorclam Coast is made-up, but motivated by some Top Knotz lore
-Barrier Idol locations are from that one Badlands map, but there are other (most of them, actually) maps that have them a little farther north. It made for a nicer southern map border when there was no Nehekhara...
Mountains
-Broken Axe Peak, made up, Grom was based around that area for a little while tho
-Karak Angkul, location is a complete guess, I just fit it in where there was space
-The Gate of Stars (Agents of the Moon's starting spot) is made-up. Although it was the Straits of Stars before it was the Straits of Nagash.
Norsca
-Lots of the Monoliths are made up, although named after real characters, and I tried to keep the monoliths in an area based on characters from nearby norscan tribes
-Several settlements based on "minor" norscan tribes. (like Blackaxes Keep, Wyrmkin Lair) Just stuck them in where there was room, mostly on the Varg coast....
-The Bjornling coast (western tip) is rock solid, all of that is from the Sea of Claws WFRP book.
-Thorshafn, from Warhammer Online
Albion
-Sherweald Forest is from a super early map of Albion, like when it didn't even look like the current iteration. (Although the more I research that exact map, the more I worry it isn't authentic, which I try to avoid.)
-Carn Mallog and Black Bog, fit them in where there was room
Kislev
-Grave of Hela Halfdead, based on a battle shown on that one norsca map
-Macadone, from Warhammer: Dark Omen, exact position is even given on a map
-Todtheim, ruins in which Azhag found the Crown of Sorcery
Zorn Uzkul
Things get a little "looser" here.
-Yatrukovsk, made up, it's next to Ozeryatruk, which sorta means "Lake Yatruk"
-Granyvsk, made up, it's next to Ozergrany, which sorta means "Lake Grany"
-Ivory Mounds, made up
-I sorta just smashed together the WFRP "Something Rotten in Kislev" version of this area with the current iteration of Zorn Uzkul. So you got Chernozavtra and Zapadryeka (yes, yes, I know it's just the river Chernozavtra is on....) and a Dolgan Camp here.
-Temple of Zemlya, SRK mentions the Dolgans worshiping Zemlya. Obviously not true in current lore, but still nice.
-Beacon of Firengrom, something from Warhammer Online
-Mingol-a-Elgal, something from Warhammer Online, went with this name rather than "Tower of the Elves"
Southlands
-Zandri stuff from AoR
-Mortis River Delta shipwrecks from The End Times: Nagash. Anachronistic, but whatever.
-Araby stuff pretty good, although moved some nomadic tribe stuff north
-There is zero lore on the Oasis of a Thousand and One Camels, it being a magical forest is made up
-Took liberties with putting Zandri AoR stuff further afield, where there was a gap to occupy. Better than just making stuff up from scratch.
-Canal of Abundance: size, exact location, direction, etc. are unspecified. It was one of the Khemrian kings who had it built, so I think that spot is reasonable. And it's probably supposed to be like the IRL Canal of the Pharaohs, and the only good direction for something like that in the Warhammer world is towards the Gulf of Medes in the south. Also there's literally nothing else that could be put in that spot, so here we are.
-Twisted Spire: Catacombs Of Terror thing
-Pools of Despair, the Grove of Thorns appears on that one nehekhara map, unlabelled. It looks planty enough. The wef faction is made up, but Sa'aali are a type of female djinn, so Arabyans would probably call Dryads something like that.
Galleons' Graveyard
-Nothing is really made up
-Islands come from the Dreadfleet rulebook, either from the 'generic' names of the island pieces, or explicit island names from the scenarios.
-Shipwrecks are all real sunken ships, although I doubt the timeline matches for some of them
-Made a few of the island's shapes based on the official map
Darklands (Minus Zorn Uzkul)
-There is surprisingly little lore on the Dark Lands. And the Dark Lands are surprisingly big.
-Lots of made up stuff. Zharr Naggrund is almost entirely made up. Well it's all Khazalid and Zharralid, so maybe not entirely made up.
-Yep
-Oil/petrol platforms, made up.
Chaos Wastes
-Lots of AoR stuff once again
-Temple of Heama is from Something Rotten in Kislev, another one of those minor gods worshiped by the Dolgan in old lore.
-Steps of Khorne are from Valkia's novel
-Hill of Bones and Sigmar's Brave are from Tome of Corruption
-Bloodrift should be way farther northeast, but ooo spooky shifting chaos influence, anything can be anywhere, oooo.
-Schwarzvolfs, their actual location is vague, they are coastal and there is probably a mountainous region around. Also from context it didn't seem like they were in Norsca itself.
-Dervingard. In the prologue, you end up at the Monolith of the Void, so it's got to be somewhere reasonably close to that. So somewhere along (or in-land) the coastal area. The spot I picked makes sense since it's right across from Sjoktraken, easy to imagine the Norse dwarfs supply/ferrying Kislevites across the sea.
-The Lost Kurgan, made it up.
Chaos Realms
-All from that one Chaos Realms map. Except for a few additions to Slaanesh's realm, which needed something extra, took those from WH3's battle map names.
-The entrances/exits to each realm are sometimes canon, other times completely random.
-Like the great tree makes sense, since it's shown growing in between tzeentch/nurgle.
-Portal of Twilight, a canon entrance to Tzeentch's realm. But I was TRICKED into thinking the canon location was Bel-Aliad (and that the Cult of Mirrors was canon). I'm usually not so gullible when it comes to wiki fancruft. But it's not that bad, the faction leader is canon, and the Portal's location is unspecified, so Bel Aliad is a fine choice.
-Paths of the Old Ones, the Cursed Jungle one is sorta made up. The Dragon Isles one makes sense, they got cut off, probably means their portal stopped working.
-Serpent's Jetty in Norsca, going to Slaanesh's realm. Made it up. But Slaanesh is the serpent , so it's a happy coincidence.
-Drifting Castle going to Slaanesh's realm. Chaos Realms map shows some connection there, so not too much of a stretch.
-I left out the Black Pit from Tzeentch's realm. It's shown on the map, but people think it's supposed to be Mirai aka Elf underworld. I sort of think it's just a generic/coincidental name, like how there's Cracked Land in Khorne's realm and in Nehekhara... (and in Naggaroth...)
Mountains of Mourn
-Blood River and Cursed Mountain. These are battles depicted on that one "top down" Chaos Wastes map. It's tricky to say where these are exactly as the Mountains of Mourn are just blank paper on the map. Trails of Hashut is a similar story.
-Quest's End, actually a song in the lore, but I think it works well as a sort of tomb or battle site.
-Granitetooth Graveyard, apparently no set location, so used to fill in the rest of the mountains. That northwest portion has nothing else going for it!
