Glowing Sand dust: Acts as an extra life of sorts, it activates Glowing Hourglass when you take lethal damage, works three times only
Spider Cider: Shot speed up, and Isaac has a low chance to creaste a small slowing creep under and enemy
Duke's Thrumpet: Spawns a random array of friendly/charmed flies aorund you
Glowdust: Isaac glows slightly and his tears too, removes curse of darkness, small chance to concuss enemies
Tech-99: Charges up a ball of energy/electricity and hwen you let go you shoot it
Used up eraser: very, very low chance to shoot a tear that erases an enemy
Tiny Satan: Small Satan head that is always behind Isaac and charges up lile Lil Brimstone and shoots a line of fire when you let go!
Chicken Drumstick: HP Up, Speed up

i might change the item names a bit to be more sinister to fit the tone of isaac more but ill be sure to credit you for the item ideas on the eid and the mod desc!
the issue with that is that it would be either too overpowered or too underpowered depending on rng so fitting it into a quality would be difficult
i feel like most items in isaac follow these
granting negatives for balance reasons - 20/20 giving damage down
straight up good - mysterious liquid
or
straight up negatives with a upside that isnt or doesnt feel "intended" judging from the players perspective - curse of the tower, cursed eye
what you described doesnt fit in any of these but that doesnt mean it couldnt make a good item
its just mostly uncharted territory (imo) and might produce items that dont feel "quite right"
tldr - could make great items but its not been done (atleast in my eyes) officially and it might take alot of refining to make it feel good to play with
