I usually play male and it used to sometimes work. Still trying mods again to see if it can be narrowed down resolutely to a mod conflict or not.
E: I'm at a loss. I tinkered around with mods, seemingly made it work, I spawned in on a new save and even auto-sorted the mods, it all worked on my male char. Then to test it I closed the game and restarted it, loaded back in and bam: Police tag on cap gone again.
Could it be something wonky with the game itself? I have texture compression on and max texture size both times on 1024 (it worked with this before).
First off: Thanks for getting back to me about this issue.
I think I finally narrowed it down after a lot of more testing - yes, B42. Turns out it may have been a mod conflict after all because on graphical settings 512 max texture and texture compression it now reliably works even after closing the game and re-loading into the same save.
Turns out that AuthenticZ broke it for me; keeping it off makes the textures and institution tags work now reliably even after game restarts. I'm sorry for throwing it out there because I did test it with the mod off but still had the issue appear at times in existing and new runs so I couldn't narrow it down to that mod specifically (maybe some wonkyness of the game in general) but now it seemingly appears to be the cause.
To get some benefit out of it I'd consider testing it yourself and / or adding a possible incompatibility issue mention with some mod textures if you use AuthenticZ (in the full version at least) enabled to your mod description.
I'm just happy I was able to narrow it down after so many new runs started and testing (I tend to be nitpicky and was somehow annoyed about the missing cap textures a lot, so it wouldn't leave me much rest haha).
TL;DR folks: Expect some missing tags/textures on some items if you use the full version of AuthenticZ; it doesn't just seemingly affect this but other mods with unique cap additions as well.
