Different projectiles (syringes)?
This bad♥♥♥ weapon needs some bad♥♥♥ syringes for ammunition. Maybe it could shoot caducei (you know that snake that goes around a staff with wings), or modified syringes that match the gun. I am also wondering if the trajectory could be different for the syringes. Tell your ideas please ☻

It could be shooting electricity ray (maybe a little bit like the short-circuit ?) like a defibrilator !

i was thinking more like a lazer, does less damage but can set enemies on fire! possibly a rage meter that when filled can penetrate through!
maybe something like the pomison
yet a quicker projectile speed, and around regular syringe damage.
maybe less damage in order to compensate for the straight shot

With a name like Brainteaser, I'm thinking the syringes might be loaded with a drug that might cause a confusion-like effect for a few (5-8) seconds. During those seconds, the victim can nolonger distinguish between friend and foe (tatget's vision becomes grey-scale and names are not displayed) and his attacks can harm his own side as well as the enemy.

A Medic with a Brainteaser could cure a teammate under an oposing Brainteaser effect by shooting him with his own Brainteaser.

To offset this, taking a visual que from the Brainteaser's appearance, it might have a reduced payload.

Maybe skringes with instead of needle they're lasers and here are the stats +Bleeding for 2 seconds - 40%- firing rate

Yeah, the team damage really puts a potential for intentional teamkilling. This idea might sound cool at first, but really kinda doesn't seem like it'd work too well after some thought.

maybe it could shoot mad milk or jarate doses or somthing, like this=
+30% reload time
+2sec. dose of mad milk/jarate (you choose)
-60% firing speed

or you could have:
+80% damage
.5sec. drugged effect on user per shot
marked for death when equipped and for a short time afterwards
doubles the healing of user while in use

OR even,.. since the name is brainteaser then maybe;
while equipped, healing gives blindness 5% of the time per minute on both the medic and the person being healed
+50% healing rate when equipped
adds a 1000 ammo capacity for the medigun total

there isn't even a drugged effect in the game he would have to implement a drugged effect pack

I was thinking:
+100% flinch (When you're zoomed in with a Sniper Rifle and you get shot except this will affect all classes now)
+Needles not affected by gravity
--50% Firing Rate

Maybe projectiles that glow like arrows but are still syringes.
