Yep, I'm having the same bug, but only when I'm starting a new city. I'm guessing the bug has to do with how the game limits your road choices until you put down a two lane street.

Same here, always happens at the start of a new map, but as the other road options are unlocked, the mod starts functioning (after a quit and re-load). Not a big issue, although perhaps some kind of check could be implemented to detect when the road options are locked.

Fix is to just put down a road. Mod author should really check if the UI element is there tbh
