The reason for this is because when you shoot a bullet, the game generates the bullet and the bullet hole. The bullet is what deals the damage, while the bullet hole is visually there. These two don't generate at exactly the same place and this is more noticeable when you are moving and shooting (you can test this by putting, sv_showimpacts 1 ,in your console). This why it looks like you hit the button but it doesn't activate.
