Not at all. The hitbox of a class is made of a bunch of boxes that fits the size of each bone of the model so the detection is as fair as possible. Only the head got a bigger hitbox to make headshots easier to hit and that's why hats aren't a problem. See here : https://youtu.be/9TJmiDcsxdA?t=87
Items that messes up with the silhouette shouldn't be accepted, even though they are gorgeous and awesome just like that one, or be Halloween restricted like most exagerated costumes. This item buffs the Medic's muscles too much for the normal game.
