emm... let me think...
can't think of any stats but it would be great if...:

+Victims don't die instantly, they are poisoned infinitely (cure: medic, hp packs,...)
+You don't lose your disguise
-15 health
-35% attack speed

those are mines ;)
Attacks deal damage to all enemies connected by a Medigun
-25% slower firing speed
This weapon holsters 15% slower

*Yes, if you backstab a Medic healing someone, you'd kill both of them, just as if you were to backstab a heal target, the Medic would die*

how about,

+Victims do not make any noise when dying.
+On hit: causes bleed to enemy for 8 seconds.
i Backstab victims spray alot of blood.
-20% slower firing speed.

I like that idea the blood spray could add a new stealth element to the game

0/10- too creative, this is a valve game we're talking about

Silent killer- no noise or indication on kill.
On backstab, give disguise of enemy killed

oh plus it like has a fancy draw animation or something

You see, i dont like the idea of removing the MAIN core element of a spy (aka his backstab). Also this knife would be more of a nuisance than an actual viable weapon... i like the idea though so that's something cx.

RIP ANY SORT OF SERVER THAT HAS 11 MEDICS AND 1 HEAVY

On backstab: Revolvers do 50% more damage for 10 seconds
Cannot disquise for 10 seconds after swinging

I love the idea of poisoning people, but it would be more balanced if a puff of smoke or something came from the spy when he did his backstab rather than nothing at all

Here is my version of stats

+Poisons players until they find health
-Doesn't kill on backstab
-random crits
