Possibly a rare drop from killing Mom? That would be quite an interesting addition to the mod. Of course, you'd need to add mom.

Moms knife could be a melee weapon that gets more powerful for every few kills, or a throwable boomerang like weapon that dose decent damage and

the knife should be the same as in the real game and you should add brimstone (that would be op but more charge up time would be good and you could craft a moms knife brimstone synergy :'D XD)

Mom's knife: a ranged weapon (like a boomerang) with no durability that increases your base damage multiplier per kill when you hold it. when you take damage while holding it, the enemy has a 25% chance to take damage of some sort :/
