["lrl"] doesnt do anything and ["lrr"] appears on the list for dark bum and leech but is for leech in game

It was a typo in the code, thanks. Should match the list here now.

rdu results in "melody too weak" even though I have the mighty one

They shuffled around a lot of item qualities and I haven't updated this list since. It's on the to-do list.
Psy Fly was bumped to Quality 4 so it would require Manifest Melody now

"ldd" says melody is too weak consume more items

This chart was made before some recent balance changes to the game, Lil Dumpy in particular was drastically changed and has a higher item quality now. I haven't found time to update this list with the items' new quality values
