I think it's because the game is trying to place City States that don't exist.
I noticed that if I select no city states during a game, it loads fine.
Once I increase the number CS in a game, the chances of the game including an invalid CS in the game increases.
For example, in my two most recent crashes, I noticed that the game was trying to place Stockholm and Singapore as CS; but I don't on the DLC that introduced those CS to the game.
Unfortunately, selecting/excluding city states from the game on the setup screen did not work to resolve this issue.
Did you ever get a chance to look at the logs? It seems to be related to city state spawning since it always spits out some errors with their placement. Most notably, I can never get Mitla/Palenque into a game, even with no Civs on the New World and Babylon screws up often as well.
I managed to get a game started after about 10 attempts by deselecting specific city states using the menu, but for all I know, it could be random chance whether or not the script bugs out. Even then, it claims that there is no TSL for Mexico City, even though there are now Civs on the New World...

Seems to be the same issue in other logs, the code stop assigning a new temp plot at some point, while there are still possible alternates in the list. a (useless) check to fertility maybe ?
