Map Name
Make sure the map your loading in gmod and the map in hammer have the exact name down to any versioning, if the bsp file is de_map_vd1.bsp, then the map must be de_map_vd1.vmf
Map Version
This one is weird, but from what I understand every time you save in hammer, the map's 'version' number will increase. This version is saved into the compiled map aswell and gplacer can access that, but if you compile the map, and then save the vmf a bunch of times, their version wont match anymore, and gmod cant connect to hammer. I get around this by trying with incrementally higher versions, but if they differ for more than 20 versions it stops looking because running the search for two long can make the game crash.
TL;DR
Try recompiling your map and gplacing the newly compiled version.
