Companions auto weapon/armor equip from loot not working

So my Riva still has a single sword as her weapon, and even though clearly better weapons/armor appear, all the companions(including Riva) seems to be just ignoring them. Does anybody have the same issue?

I usually just take away the other weapons. They main which ever i give them so theres no switching around during fights. Never had them switching armor up tho. They alwasy wore what i gave them. Dont have them take from loot during battles. Do it manually.
