About the ghost. (!!!)
So, im sure it is a REAL ghost. It's not an error. This ghost will disappear if you walk the stairs (there's a trigger somewhere, hard to tell where since my english is crap) so it is impossible to see it without noclip. Though you can noclip there and cleanup the map via admin menu (in "utility" category) and you will see it. It is not a texture, it's an actual model.

As I said it disappears as you step on the stairs. You know, there is 4 floors, 4th being abandoned. Right there you can see the ghost. It disappears as you walk on 3rd floor, so when you will be on the stairs between 1 and 2 floor you can see it floating up there.

To view triggers, type showtriggers_toggle in the console after you enable cheats.

Its been a while but yeah, there are several triggers around there that make the ghost disappear. When you step on the stairs at the lowest level you can hear some kind of high pitched singing as well. The easiest way to see it is to get up on the first level and then noclip all the way up. It looks like a black stain.
