However, starting from +3 to +4 (and all subsequent upgrades from +4 and up), the mechanics change. There is indeed a 33% Success Rate, and a failure will result in a downgrade.
So, when you attempted to upgrade from +3 to +4 and it failed, your weapon correctly downgraded back to +2 according to these rules. It's a challenging system designed to add a layer of risk and reward to higher-tier enhancements.
