One time I completed an ethereal map and it unlocked a secret level "Hadron Collider" which offered 2 rewards. Unfortunately, I had to fix / restart steam and in order to do so quit out of the map and restart the game. When I loaded back up the map and my chance to complete it was gone.
So I'm left wondering whether that map is necessary.

Is it pure luck to get the ethereal brigde maps, or can I influence it?

Advice to newbies: Do it right from the start!
So there's a way to get lots of maps - FAST - that avoids all that horrific grind. I'd played the game pretty much to completion and got barely any maps before I found out about these hidden levels so I get the frustration of everyone here.
I found a custom map - basically a tier 21 map with 99 waves where every wave has 100 horsemen and the path pretty much guarantees they'll fall into lava if you follow the instructions. Workshop maps don't count towards xp (or achievement progress I think) but you do get to keep the items, and running this map properly netted me roughly 12 full pages of items with 6-15 maps each time.
I put down a few teleport runs and traps near the end of the path to make sure nothing got through.
Hope that helps :) I just got all the achievements thanks to this so can confirm it works.
Anyhoo.... long story short, Old Road and Mang will not appear for me at all, no matter what I do. I got every other map but Old Road and Mang won't appear at all despite having the correct linking maps. I've cleared the linking maps several times, nothing. Oh well.
edit: I'm level 2500~ in A13 and clearing the level 100+ ethereal maps (several with 400+ waves) to do this was annoyingly boring. I agree with the point earlier -- definitely do it early. Assuming it still works.
edit: I finally got Mang to appear just now but the other one, Old Road, remains stubbornly hidden. No idea why.
edit2: Old Road is next to Mang on my map (one square higher up), it is not where it was on your map posted earlier. It appeared after I cleared Mang.
edit3: Ok got it and Reaper of Objectives which I also didn't know existed apparently I have 100% achievements in this game now since apparently those were the only 2 I didn't already have. On to A14, and thanks for the map screenshots earlier, they were definitely of some help in making sure I tagged them all!
