If I may make an interesting note, you could always reverse the effects of Guppy's Paw and create a full bone heart out of three soul / black hearts?
I do want to preface that this is, of course, a mod made at your discretion, not necessarily something that has to be perfectly balanced to match with the rest of the game.
You have a nice thematic, a good idea, making use of how a couple of existing items in the game work and creating something out of it.
If you have the idea in mind to create something that you want to create, then you make it exactly how you want it to be. Balance this however you want to, because it's yours to mess around with, y'know?
Using Guppy's Paw is a gamble because you trade one permanent heart container for three finite soul hearts. Good short term, bad long term.
With this item, there is zero excuse not to use it immediately and as frequently as possible.
Just providing a little criticism. It's a very nice idea, I just think you may have picked a difficult thing to balance.
