the issue is due to the unlimited guardians mod. Had the same happen to me when I tried this mod out.

1.Launch OMD 2
2.Uncheck (Inactivate) all mods (Reason: There is a possibility that another mod is cancel each other.)
3.Check (Activate) Ball and Chain Guardian(Charms nearby orcs)

for some reason when I want to spawn ball and chain I spawn normal paladin instead. When I choose the location of it it shows as if it was ball and chain but when I actually set it up, paladin spawns. Any idea?
