That was just an edit to the video, although I'm sure one exists somewhere that does that in-game.

Make sure you don't have any addons that mess with it playing. Anything that changes the sounds of the player dying will likely mess with it; for example other death sound addons.

when you spend hours working on a super awesome dupe, but accidently press R (unweld all) on it before you can save it.
