I think you just have to get the files again, then go to \Documents\Paradox Interactive\Hearts of Iron IV\mod and add them, make sure to rename folder as it's named before or as the mods name here.
Go into the mod's folder and grab the descriptor.mod from inside it and drop it back in to \Hearts of Iron IV\mod and rename it to the mod folder's name.
Go inside and add path="[path here without [] ]"
Example:
path="C:\Users\adept\Documents\Paradox Interactive\Hearts of Iron IV\mod\testmod"
Of course, you need the remote_file_id and everything else.
If you can't get the .mod file or your mods files, sub to the mod and go to \steamapps\workshop\content\394360\2359072903 (i.e your mods id) and you should find everything relating to your mod at the update of the workshop.
I'm not really a modder but I hope that works, I can try to look in the paradox forums or reddit if it doesn't work
