If only there was a screenshot in here that showed a pic of Encyclopedia for the item showing how it only appears in the Secret and Ultra Secret room pools

I've had this mod installed for 1 - 2 month or so now but haven't seen it until now... have a lot of item mods active but also play a lot of Isaac... What pools is this item in?
