It would be great if you could add a convar to adjust general volume AND/OR scale volume based on damage taken. I do combine your wonderful addon with EFT Shot FX to get even more diverse audio and visual hit feedback (fully turned of the native red screen sauce and tinitus as well, so this is all feedback you get when hit). Together they really make you duck and cover as soon as bullets hit you, it is really frightening. Also logical, as one can not proper aim while bullets hit you.
So far, so good, it is just a bit[..] to loud, especially since there is no audible difference between getting 2 health damage and 50.
Also, that headshot from CS:GO sucks ass, I'd also want it gone.
