American Civil War Fail Condition

Currently the journal entry for the American Civil War can't be failed. Due to what I can only assume to be an oversight, if the USA loses the civil war the journal entry just remains there, only going away if the USA reconquers the entirety of the CSA or FSA, with the reconstruction events happening afterwards.

This mod simply adds a fail condition to the journal entry, making it go away if the USA loses the war. As a consequence of this, the reconstruction events will only happen if the USA wins the war, and most of the dynamic flags for the CSA will appear ingame (they require the journal entry to no longer be active, so they don't appear in Vanilla).
Additionaly, I've tweaked the flag definitions for the CSA since some of the triggers didn't make much sense to me. (this feature has now been moved to another mod)
