This mod don't use periodic random spawn, only spawn npcs if triggered by player actions using the walkie-talkie.
When you use "Call Survivors" to spawn friendly npcs there is a chance to spawn enemies instead.
About the issue ... I'm not aware of, but I will look into it. I'm pretty sure that there is no chance of "ghost npcs", sometimes the npc break your clothing, if they break your boots then you take damage.
Please check the settings of TakeNoPrisoners mod, there are options to control the spawn frequency and friendly spawn chance. Recommend to change to defaults.
