It should only appear in the starting room after first installing the mod. It's seems there's a problem with saving for you. I'll look into it

@bloom the hotheads die when you extinguish all fireplaces

the hot head enemys dont die after clearing the room while you cant damage them+

I have the impression that the main effect of Caecus doesn't really work.
If you're already discovering items, the consumable doesn't reroll, but I do get the impression that the secondary effect works.

It seems to occur with the secret room that has 2 drawings set to appear in the upper left and bottom right corners with spider webs around them.
