Before we get into this, please know that pills gives you up to 50 temporary (temp) health points (hp).
1 When at low health
Not at 60 hp, not at 50 hp, not when a teammate tosses you pills and you pop em once you're at 80 hp. Use them ONLY when you're in the red zone.

2 When a tank spawns
Use your pills at 40 hp once the tank spawns. You're most likely gonna get shot during a tank encounter and that's VERY dangerous at 40 hp, because one solid hit from a teammate and you can get dropped to 39 making your speed slow. So go ahead and pop those pills in on expert or advanced at 40 hp once that tank spawns.

3 When you see an extra set of pills
Please disregard the first listed tip for this one. If you're at 50 hp and you have pills while everyone else is at 80, 70 hp with full slots, just pop the pills in. However, if you see 1 more after you grabbed the next one, please don't be that guy to use the pills again and grab the last one. Let that 70 hp player know that their's an extra one for them.

4 Before an event
If you don't have a shot (which I will get into after this section), use your pills at 40 hp. You will have to keep your default speed AND be able to sponge up all that damage your teammates may accidently dish at you or the infected.

In this section, I will shorten the name to simply "shots".

Before we get into this, please know that the adrenaline shots will give you up to 25 temp hp and a speed boost.

1 Before panic events/running events
These events need you to constantly move, because the hordes are endless and you need to get to a certain point to stop the horde, whether it is the alarm objective or saferoom objective. You're gonna need a speed boost to get through it quickly. So use it right at the event, not before it.

2 3 players are incapacitated (incapped)
In an expert game? Versus game? The shot can help you easily clutch the whole game. Remember when I said the shot gives you a speed boost? This doesn't only apply for running, it applies for different actions such as healing, starting up generators, fueling cars, or in this case helping your fallen teammates back on their feet.

The actions get cut by 2 seconds (5 seconds default, 3 seconds in adrenaline effect). Use it to your advantage.

3 During a horde and you're black and white
I know what you're thinking. "But you just said don't use pills at black and white!"

Yes, but notice how I said pills. Adrenaline shots can be way more effective than pills, depending on the situation you're in.

Let's say that you're on expert, you're black and white and you need to heal up. But all of a sudden, a huge horde rushes at your team and you're in big trouble. That can all change with one adrenaline shot. Use it, run to a safer spot, heal up, and join in the fight with your team. An adrenaline shot can save your life. Don't forget that.
