Download and install Eye of The Storm to sourcemods which is located in:
Steam\steamapps\sourcemods
If you are on the Synergy 4.3 beta, you don't have to do any additional steps other than subscribe to the addon.
Then copy the folder called 783933738 located in:
Steam\steamapps\workshop\content\17520
and paste in: Steam\steamapps\common\Synergy\synergy\custom
If you have Synergy installed in the default directory and are running a 64-bit computer, you can use this command in cmd.exe to make it easier:
^ That is meant to be on one line, copy it, then right click on the cmd.exe window, click paste and press enter.

Right click on the finder icon and click "Go To Folder" and type in:
In this folder you will find common, sourcemods, and workshop. Download and extract Eye of The Storm to sourcemods, download this addon and copy the folder in workshop/content/17520 under the name "783933738" to: common/Synergy/synergy/custom and when you start up Synergy, you should see Eye of The Storm in Create Server and also be able to join servers that host it.

If you are on the Synergy beta 4.3, you can now add 783933738 to your Synergy\synergy\cfg\workshop_srv.cfg
If you don't have workshop_srv.cfg, just create a new text document and rename it to that.
The content tag and first map is: "eots eots_101"
