Updated the volume of the theme song to be 5dB higher. It's still not quite as loud as I'd like it to be, but going higher will make the audio clipping more obvious.

Between the release of Quality of Death & v4.0.2, the Death.ogg file was unable to recognize changes so this mod was broken for that span of time. It should work fine now, but if there's any future problems leave a comment and I'll take a look.
