Updated mod to work with new Koth plugin

*fixed bug causing crash when using points removal on death option
*added option to show koth block gridname and planet in scoring chat message
- option can be enabled in block UI via checkbox for location name

** If issue occur on boot delete scores.data and it should fix problem
