Updated to be compatible with e0.0.44+.
Added - intake pressure estimator.
Added - automatic reference cylinder size identification.
Added - automatic selection of internal (estimator) or external intake pressure source.
Fixed - PID integrator no longer can keep engine runing after disabling an engine.
Removed - removed AFR estimator from MC.
