Tweaked how the boss rush door is spawned, should be less janky now and not appear before the fight is over

Boss rush/hush doors stay open even if you leave the boss room

Fixed a bug that made the mod only work correctly once per boot of the game. If you were late to the boss rush and the mod spawned the door anyways for you, it wouldn't spawn the door late again on subsequent runs until you restarted the game.
