- Changed "Time Elapsed Since Combat" to "Time Elapsed Since Peak Stress" as part of the transition mechanic for the Madness Combat State, and changed it so that it only starts counting the time after you've hit "peak" stress which is currently 85 so as to be lenient.

- Fixed typo in the explosion factor code.

- Refactored enemy proximity factor to have exponential scaling based on distance. This should make close enemies amp up the stress faster than enemies farther away.

- Added a 3x multiplier to the enemy proximity factor for if they are targeting the player, this should make transitions from calm to combat more immersive since they don't just see you but are also targeting you.
