NT Surgery Version 1.3.0

- Added a small initial boost to surgery skill (20 - 25) for Medical Doctors so they can perform basic surgery from the start
- Changed file & directory structure to be more in line with the vanilla game files

Balancing and QoL update
Changes
- Medical doctor changes to starting items were removed
- Surgeon changes to starting items: removed item kits
- Reduced mod size by removing asset bloat
- Mod files were organized

Added
- Surgeon id card fix by Torca. Surgeons can access the same restricted containers as medical doctors.

More changes (and maybe reverse changes) to come around
