I think to fit the theme, the amnesia should occur everytime isaac gets hit. If you can't wipe the map, then the usual amnesia on 1 heart damage, or 2, or on death, depending on the balance.
How does it interact with other sources of lives? (So I can be sure any interact with other lives is intented or a bug)

2, 3, 5, 8, 13, 21, 34, 55, 89, 144, 233
so even if you have a ridiculous amount of items, you won't be able to resurrect more than ~13 times without picking up more inbetween deaths. (Unless you spend hours getting thousands of breakfasts ;D)
