There's a mod of this here http://mods.tf/mods/1507/ to install it pick what hat you want to replace and put the vpk file in the custom file. But remember, it doesn't work in valve servers or any server that has sv_pure 1

It's probably a refrence to super mario odesessy, specifically a character named cappy, whose true form is a ghost with a white top hat as it's upper body, with eyes on the front of the hat, it replaces mario's hat's at the start of the game, so, i think this is a a pun on cappy acting as a replacement hat for mario as well as it's true form
