Go to your Steam folder, steamapps, workshop, content and search for this item's ID, 1256224338. Make a copy of it in DST's mods folder and go at it.
You'll also need some tools. And maybe even some reference materials.

Though, I think the guy behind the API is working on it.
Y'see, when modding the character's portrait you have to name it a certian way in a document. The problem is when I do that like how I'm supposed to, when the portait needs to be called up, the whole game crashes! So I end up leaving it alone for that reason.
The thing is, when the portaits aren't named properly in the document, the game crashes when going into a cave. So I'm left to pick between two poisons. Trust me, I would more than love to fix that problem, but I can't!
If you wanna play with cave items, get the megarandom mod.
