Drag and drop the .bsp file into your garrysmod/garrysmod/maps folder. it'll be added to the map selection menu automatically.

The map is mds-mcdonalds. It's not on the workshop yet, but it is on garrysmod.org . Just do a search for "mcdonalds" (no comma) and you should find it.
