No worries mate, your're not dumb, we all had to bang our heads to the table at one point or another :)
1. Could be the particles limit but i doubt it unless you use too many particle systems at the same time. Make sure the script is executed, place a hint in it and see if the point where the particles are created is reached. If is a MP mission you have to consider the locality. If remember correctly i think the easiest way will be to execute the script via init
2. Hmm, the script is pretty old, i think i used the waituntil which is not the friendlier thing to do performance wise, and in the last years waituntil command was subject of change which can bring some issues. If you have the same problem in demo mission it means the script needs a bit of rewriting
1. The Ash fallout isnt appearing, it appears in your test scenario but I cant get it to appear in my mission
2. The geigor counter when enterring in radioactive area is going ballistic and makes the worse noise possible rather than the nice ticking noise

Yes, but not the way the script is setup, basically it has to be rewritten for that. Have you tried the radiation script? Maybe you can use that one.
It shouldn't, people are using it in all sort of projects from missions to all sort of persistent game mods.
It can be done, but there are 12 parameters to tweak and without an editor is difficult. Look for a mod or a post-process editor for arma, there is one out there but i don't remember the name.
