For the heir problem, I personally haven't had a problem with it. I think maybe one in ten actually die without an heir of age, and every time they do they usually have a kid I can pass the title to. This happens maybe once every couple of decades, so personally I don't see it as an issue.
Also, generate wives/husbands and children for all the characters, since a lot of them die in a few years heirless and the titles end up reverting to the liege.
