Hello, I noticed that the w_model's flashlight attachment is still in default position making it not match the location of flashlight. You can fix this by viewing the model in HLMV and adjusting the translation and copying the qc line into the qc file. I think this should be good enough?:
$attachment "flashlight" "ValveBiped.weapon_bone" 0.00 3.00 15.00 rotate -0 0 0
